During the treatment scene, how is the spectator punished?

During this scene, Alex is forced to watch the 'viddies' much like how the spectator is forced to watch (and often be implicit with) his actions.
Initially Alex enjoys the films and we, as we are aligned with Alex, enjoy them too. This coincides with how during the first half of the film Alex enjoys committing acts such as rape and beatings and the spectator enjoys watching Alex do so.
However, as we see Alex start to feel uncomfortable and ill, the camera tracks forward to frame him in a close-up that the spectator is forced to view.
